Schweiz Med Forum 2007;7:158–161. - Hiccup (singultus) continues to fascinate and confuse physicians and patients alike. It results from involuntary transient contractions of the diaphragm while the glottis is spasmodically closed. The contractions of the diaphragm are usually evoked by irritation of the phrenic nerve by various aetiological factors or by damage to inhibiting regions of the central nervous system. Many cases of centrally generated hiccup are still misdiagnosed as idiopathic or psychogenic.
Nervenarzt 2005 · 76:756–759. Treatment of psychiatric patients often necessitates overlapping neuroleptic medication. We report a 60-year-old woman suffering from a schizoaffective disorder who received temporarily three neuroleptics, together with lithium. She developed neurotoxic encephalopathy with symptoms of a malignant neuroleptic syndrome. It is unclear if irreversible brain damage will remain. We recommend frequent electroencephalographic controls for early detection of neurotoxicity.

In vulnerable individuals, dose-independent ecstasy abuse can lead to unpredictable and potentially dangerous neuropsychiatric sequelae which require proper initial assessment and adequate treatment. Atypical psychosis and complex-partial seizures of ecstasy use are discussed. Evaluation of possible evidence for toxicity–vulnerability predictors and implications for preventative and clinical care.

The second messanger Ca 2+ is known to act in a broad spectrum of fundamental cell processes, including modifications of cell shape and motility, through the intermediary of intracellular calcium-binding proteins. The possible impact of the lack of the intracellular soluble Ca2+ binding proteins parvalbumin and calbindin D-28k was tested on spine morphology and topology in Purkinje cell dendrites of genetically modified mice. The results suggest a morphological compensation for the lack of the soluble calcium buffers in the cytoplasm of Purkinje cell dendritic spines. The increase in various spine parameters, particularly volume, may counteract the lack of the calcium buffers, such as to adjust Ca2+-transients at the transitional zone between spines and dendrites.

SAEZ Schweizerische Aerztezeitung 2009;90:43. Nach mehrjähriger Entwicklungsarbeit kann die Stiftung EQUAM drei Zertifizierungssets mit Indikatoren zur Performance-, Ergebnis- und Indikationsqualität vorweisen. Erstmals haben sich nun im Sommer 2009 drei Grundversorger einem Zertifizierungsaudit zu den Minimal Standards der Diagnose Diabetes mellitus unterzogen und dazu die innovative Softwareeigenentwicklung Diabetes Databox des Ärztenetzes mediX zürich genutzt.
Care Management 01/2008. - Der Diabetes mellitus hat sich mittlerweile zu einer Pandemie ausgeweitet. Weltweit leiden gemäss Schätzungen der Weltgesundheitsorganisation WHO über 150 Millionen an dieser Erkrankung. Für das Jahr 2010 wird ein Anstieg auf 240 Millionen Menschen prognostiziert. Schätzungen der international Diabetes Federation (iDF) gehen auf globaler Ebene von einer Verdoppelung der Zahl der Diabetes-Betroffenen in den kommenden 20 Jahren aus. Davon werden etwa 90% Typ-2-Diabetiker sein.
Persönliche Impressionen vom «Internationalem Forum für Qualität und Sicherheit im Gesundheitswesen» in Berlin 2009. Im März 2009 fand das äusserst anregende und vom BMJ getragene «Internationale Forum zu Qualität und Sicherheit im Gesundheitswesen» erstmals im «Internationalen Congress Centrum» (ICC) in Berlin statt. Die Ursprünge dieses Forums gehen in die Vereinigten Staaten zurück, wo 1989 erste Veranstaltungen zum «Quality Movement» über das heute hochrenommierte Institute for Healthcare Improvement organisiert wurden, dessen ausgezeichneter Website (www.ihi.org) in jedem Fall einen Besuch wert ist.
Care Management 06/2009. - Die Qualität einer medizinischen Leistung zu visualisieren und standardisieren ist eine anspruchsvolle Aufgabe, mit der vermehrt auch Hausärzte konfrontiert werden. mediX zürich startete mit dem Pilotprojekt «Databox» deshalb einen Versuch, die Behandlungsergebnisse bei Diabeteserkrankungen zu messen. Dank den Resultaten dieser Studie konnten bereits Verbesserungen im Behandlungsprozess implementiert werden.
Kopfschmerzen sind häufig in der Bevölkerung. Fast alle Menschen leiden im Verlauf des Lebens ab und zu unter Cephalaea, die meisten unter Spannungskopfschmerzen oder Migräne. Etwa 90 Prozent der Bevölkerung hat mindestens einmal pro Jahr Kopfschmerzen. Kopfschmerzen sind aber nicht einfach nur Schicksal. Man kann die meisten Formen erfolgreich behandeln und den Anfällen auch vorbeugen.
Qualitätsarbeit hat sich in der Schweizer Medizin etabliert; dies aufzuzeigen, hat sich die Arbeitsgruppe Qualität der FMH zur Aufgabe gemacht. Deshalb stellt sie den FMH-Mitgliedern in vier aufeinanderfolgenden Ausgaben der Schweizerischen Ärztezeitung Qualitätsinitiativen vor, die Referenten aus verschiedenen Fachgebieten und Arbeitsbereichen (ambulant/stationär) präsentiert haben. Zugleich publiziert die FMH- Abteilung Daten, Demographie und Qualität DDQ diese Kurzporträts auf www.fmh.ch und ergänzt sie online um praktische Informationen. Die Sommerserie schliesst mit dem Porträt über Databox; dieses Projekt will die Qualität der ambulanten Betreuung von Diabetespatienten steigern.
Experimental & Clinical Cardiology 2014: Evidence-performance-gaps between guidelines and treatment of type 2 diabetes (T2DM) in daily practice have been reported, especially in primary care. We aimed to assess the potential gap comparing current treatment in primary care with guidelines and patients’ characteristics from large clinical trials that guidelines are based on, namely UKPDS, ACCCORD, ADVANCE, STENO-2 and VADT. Methods: We undertook a cross-sectional study and extracted data on 541 patients with T2DM from a clinical information system of a GP network in Switzerland. Results: Our study population was comparable to patients in ACCORD, ADVANCE and VADT at baseline. Patients in UKPDS and STENO-2 differed in age and disease duration. HbA1c-levels (7.3%), LDL- level (2.6 mmol/l), systolic and diastolic (135/78 mmHg) blood pressure were lower in our study than in the reference studies. 39.4% received an ACE-inhibitor, 41.6% statins and 41.4% aspirin. Conclusion: Taking into consideration the results of recent large clinical trials indicating that very strict treatment goals are of no additional benefit, most patients in Swiss primary care would not benefit from a treatment intensification regarding HbA1c, blood pressure and cholesterol targets. Evidence- performance-gaps were observed mainly concerning the choice of first line medication.
Patients with chronic illnesses like diabetes mellitus benefit from care following the concept of the Chronic Care Model. To improve quality and to be responsive to patients’ needs reliable data on patients’ view of care in different healthcare settings are required. We evaluated the congruency of diabetes care with the Chronic Care Model between managed and non-managed care organisations from a patient’s perspective. METHODS: We compared type 2 diabetes patients from non-managed care with a managed care organisation in Switzerland. We evaluated differences between these settings with the Patient Assessment of Chronic Illness Care 5A questionnaire (PACIC 5A; scale from 1–5) that combines the PACIC and the 5A-approach of physicians’ counselling. RESULTS: 374 patients completed the PACIC 5A (326 from non-managed care settings, 48 from managed care). The adjusted average PACIC summary score was 3.18 in the non-managed care compared to 3.49 in the managed care sample (p = 0.046). Managed care patients scored sig- nificantly higher in the subscales goal setting (2.86 vs 3.29; p = 0.015), advice (3.23 vs 3.64; p = 0.014), assist (2.98 vs 3.44; p = 0.016) and arrange (2.50 vs 2.88; p = 0.049). CONCLUSIONS: Our data from different health care set- tings suggest that managed care is recognised by type 2 diabetes patients as care that is more congruent with the Chronic Care Model and offers more intense behavioural counselling and self-management support compared with usual primary care in Switzerland. Future research should evaluate larger, more comparable patient groups.
